How Northern Beaches Chiropractic handles your personal and health information.

Northern Beaches Chiropractic(“we”, “us”) is committed to protecting your privacy. This policy explains how we collect, use, disclose and store personal information, and it is designed to comply with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the Health Records legislation applicable in Western Australia.

Information we collect

As a healthcare provider we collect information that is reasonably necessary to provide safe and appropriate care. This may include:

  • Name, date of birth, address, phone number and email address
  • Emergency contact details
  • Health information, including your presenting complaint, relevant medical history, medications, imaging reports, assessment findings and treatment records
  • Private health fund and payment details where relevant
  • Correspondence with other health practitioners involved in your care

Through this website we may also collect technical information such as your IP address, browser type, pages visited and referring URL, where analytics tools are enabled.

How we collect it

Directly from you — at your appointment, through intake forms, by phone or by email. We may also receive information from your GP, another health practitioner, a radiology provider or your private health fund, with your consent or where permitted by law.

  • To meet our legal, professional and insurance obligations

Disclosure

We do not sell your personal information. We may disclose it to other health practitioners involved in your care with your consent, to your health fund for claiming, to our practice management and IT providers under confidentiality obligations, and where disclosure is required or authorised by law.

Storage and security

Clinical records are held in secure practice management systems with access limited to authorised personnel. Health records are retained for the period required by law — generally at least seven years from the last entry for adults, and until age 25 for records created while a patient was a child.

Website analytics and cookies

Where analytics or advertising tools are enabled on this website, they may set cookies and collect usage data. These tools are configured through environment settings and are not active unless explicitly enabled. You can block or delete cookies through your browser settings.

Accessing and correcting your information

You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate. Requests should be made in writing. We may need to verify your identity, and in limited circumstances permitted by law we may decline a request and will explain why.
